Health insurance card

• Soon after registering for ZEM Polis, you will receive a personal health insurance card, free of charge.
• Care providers use this card to verify that you are insured.
• A health insurance card remains valid for 5 years, and you will automatically receive a new one well before the old one expires.
• We recommend that you always carry your health insurance card with you.

Info on the back of your health insurance card

• the telephone number for emergencies
• your personal details
• the UZOVI code identifying ZEM
• the expiry date
• a health insurance certificate (EHIC) for use in a number of other countries
