Claiming the costs of care
Do you wish to claim the care costs that you have incurred? You can arrange the claim via your employer if you have joined a group insurance policy with your employer. What you need to do yourself:
1. Download and complete the Claim Form Medical Costs incurred in The Netherlands
2. Enclose the original invoice
3. Give these documents to your employer
